Personal Health Record

Healthgram Personal Health Record (PHR) is a secure online tool used to collect and store a member's medical history and test results on a personalized and encrypted website.  It is accessible around the clock via the Internet, where it can be updated manually by the member or by Healthgram.  Data from health risk assessments and electronic lab feeds populate the member's PHR, which provides real-time access to this important health information.

A special feature of Healthgram PHR is the visual display of a patient's health information.  A quick glance at the green, yellow, and red traffic light icons helps a member better understand health risks and make informed choices when seeking care.  This simple visual aid also enhances communication with our ever-growing, diverse membership. In addition, Healthgram PHR offers a wide variety of articles and information from trusted sources. The PHR helps employees understand their chronic conditions and the treatment and lifestyle changes that lead to healthier lives.
